IF YOU HAVE A STUDENT FOR THE 2023 YEAR THAT NEEDS TRANSPORTATION, PLEASE DO NOT COMPLETE THIS FORM UNTIL AFTER JULY 1, 2023. Please complete this form for EACH student. If you have multiple children you will need to complete one form for each student. Please complete this form if you are new to the District and have not ridden the bus before. Please complete this form IF YOU ARE CHANGING SCHOOLS (example - 5th Grade to Bothwell or 8th Grade to High School). Please complete this form if you are now assigned to a bus but no longer need transportation.
